For other insurance providers, clients are responsible for service fees, which will be discussed at the time of our initial communication.

There are a limited number of clients for which services can be provided on a sliding scale. Details can be discussed during your initial consultation.

A portion of the fee for services may be reimbursable through a client's "out of network" insurance coverage. We will provide clients with a detailed receipt that can be provided to insurance companies in order to see reimbursement as stipulated under your insurance plan coverage contact.

With the exception of CareFirst BlueCross BlueShield and Cigna clients, Phoenix Wellness Services LLC is not responsible for submitting insurance claims, nor are we responsible for communicating with your insurance company directly.
